For information regarding Prescription Drug Reimbursement Benefits, please call 651-256-1800 or refer to your copy of the Summary Plan Description/Plan Document. 

TIP: For fastest service of prescription drug reimbursement claims, please utilize the appropriate “Prescription/ Vision Claim” envelope pertaining to prescription drug reimbursements.  Contact Zenith Administrators or your Laborers local union to receive additional envelopes.
